Application for Dog Licence

This form may take you 10 minutes to fill in.

You will need the following information to fill in the form:

• The particulars of the dog including breed, colour, sex, date of birth, microchip number (if applicable) and sterilisation certificate number (if applicable).

You will also need to submit the following documents:

• Owner’s NRIC (front and back) / passport and working permit
• Parent/guardian’s NRIC (front and back) if you are below 18 years of age (if applicable)
• Sterilisation certificate if your dog is sterilized (if applicable)
• Microchip barcode if your dog is microchipped (if applicable)
• Please also note that for (a) passport holder or (b) if the address on your NRIC is not the same as where the dog is kept, you will need one of the following as supporting document:

1. A copy of the property tax,
2. A copy of the tenancy agreement,
3. A copy of proof of purchase,
4. A copy of the latest power supply bill, or
5. An authorisation letter from the owner of the premises, together with his/her NRIC

Enquiry on Application Status / Make online payment / renewing dog licence / cancelling dog licence / replacement of of certified true copy of the licence

You will also need to submit the following documents:

• Dog Licence number /application number
• Owner's NRIC / passport number
