A veterinary certificate from AVA will be required if the destination country of your animals/birds requires a government health certificate; or in the case of animal feed/feed premix/additives, specific certification relating to the manufacturer or product is required.
This form may take you 20 minutes to fill in You will need the following information to fill in the form
- Consignment details.
- Importer/Exporter details.
- Softcopy of specimen certification.
- Softcopy of letter of authorization if a representative/agent

Important note
Please allow 1 working day (for government health certificate); or 2 working days (for veterinary certificate for export of animal feed/feed premix/additives), from date of receipt of complete application by this office, to process your application.
The certificate fee is $23 per certificate. If the certificate is required within 1 or 2 working days respectively, an express service fee of an amount equivalent to the certificate fee will be charged, in addition to the certificate fee.
It is the responsibility of the applicant to ensure that all import and export requirements are met. Applicants are advised to enquire from the destination country on the type of certification required as any incomplete or incorrect submission may result in a delay in the processing time.
If you are submitting the supporting documents online, please get them ready in soft copy format. Alternatively, you may submit them offline (i.e. via fax, over the counter).
